At Florida Digestive Health Specialists (FDHS), we want to make sure that our patients have access to the care they need. This is why we accept most health insurance plans. To ensure that your visit is covered by your insurance, we recommend that you contact your health insurance provider prior to your appointment. This will allow you to confirm covered services and ensure that your FDHS provider is in your insurance network. In addition, we kindly ask that you bring your insurance card(s) with you to every visit to help us better serve you. By working together, we can ensure that you receive the best possible care at FDHS.

Medicare Patients:

As providers who participate in Medicare, we will take care of filing claims for you. As per Medicare’s guidelines, once you have met your deductible, you will be responsible for a 20% co-insurance. We will file a secondary insurance plan as well unless you inform us that you do not want us to do so.

Managed Care/Private Insurance 

For the convenience of our patients, we accept most major managed care plans, including HMOs and PPOs. If your insurance plan requires a referral or pre-authorization before your visit or treatment, please make sure that it is completed with the insurance provider or primary care physician before your appointment. Otherwise, you will be held responsible for the entire cost of the visit or procedure.

Patient Financial Responsibility  

Patients are responsible for paying co-pays, co-insurance, deductible payments, and self-pay fees at the time of service. For your convenience, we accept most major credit cards, cash, and personal checks.
