Will I have a new doctor?
No, our providers, staff, locations and high-quality care will remain the same.
Will my patient portal change?
Yes, all patients will need to create a username and password for our new patient portal. If you have not received an email to set up your login information, please contact your physicians’ office. If you are a new patient,
click here to “request an invitation”.
Our patient portal also has an app called Healow for on-the-go, easy access. You will use the same username and password to log in
- STEP 1: Download the Healow app
- STEP 2: Open the app and search your practice by center code: FJDACD
- STEP 3: Enter your portal username and password to log in.
I have an upcoming appointment scheduled. Will this change?
Your appointment will not change. We look forward to seeing you! If the appointment is after April 1, you will be prompted to set up your new patient portal account and fill out new patient forms (via text or on an iPad in the office).
